Licensing Program Analyst (LPA) Jennifer Velasco conducted case management visit due to notification of incoming Unusual Incident Report (UIR) by Facility Representative Jessica Stribik (Director), who stated one of the buses was vandalized and is not operable, one bus needs mechanical repair, and one bus has a broken alarm. Director stated the facility is currently only in need of three of the remaining operable buses for field trips, and all buses in need of repair are being repaired and will not be used until repairs are completed and verified.

No citations were issued as a result of this case management inspection. Exit interview was conducted and this report was reviewed with Director Jessica Stribik. Notice of Site visit was given and shall remain posted where visible to parents for 30 days.
