13 | Licensing Program Analyst Jennifer Velasco (LPA) met with Facility Representative Kimberly Iverson (D1) to deliver findings. LPA toured the facility, including all activity and classroom spaces, restrooms, food service and outdoor play areas. Census included 47 preschool children being supervised by five classroom staff. D1 was reminded never to exceed the conditions, limitations, and capacity specified on the license. Facility hours of operation are Monday through Friday from 6:00 AM to 6:30 PM.
It was alleged staff hit a child (C1), C1 sustained a bruise in care due to lack of supervision, and staff did not report incident to authorized representative. During the investigation, LPA observed staff provide care to children, conducted interviews, and reviewed documents. Witness statements, LPA observation, and facility documentation failed to corroborate the allegations that staff hit a child, that a child sustained a bruise in care due to lack of supervision, and staff did not report incident to appropriate parties. Though the alleged violations may have happened, the preponderance of evidence standard has not been met; therefore, they are unsubstantiated. An exit interview was conducted, and a notice of site visit provided. Notice of site visit shall remain posted for 30 days.
