On 02/28/2023, Licensing Program Analyst Katy Maestas (LPA) conducted an unannounced field visit to deliver the findings for the above allegation. LPA arrived at the facility and was granted entrance. LPA met with Director, Bekah Scovel (D1). LPA toured the facility and observed12 preschool aged children being supervised by 1 adult. LPA accessed Guardian to ensure that all required adults were background cleared and associated to the license.
Throughout the course of the investigation, LPA conducted observations, interviews, reviewed and collected documents. It was alleged that a lack of supervision lead to children engaging in inappropriate interactions. Interviews revealed that 2 children were hiding under a table without their teacher’s knowledge and engaged in inappropriate interactions.
Based on interviews, file reviews, and observations conducted the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. A Type-A deficiency was cited on a subsequent 9099-D page.

D1 incorporated a policy for more table top activities for opening and closing times of the day. New toys were purchased for these activities. The classroom was rearranged to reduce blind spots and staff were provided training for supervision on 02/20/2023. D1 will email LPA a list of staff who attended the

This requirement was not met as evidenced by a teacher not having visual observation of 2 children on 01/17/2023 which resulted in the children hiding under a table and engaging in inappropriate interactions.

D1 understands that all parents or authorized representatives of enrolled children under the preschool license are required to sign the LIC 9224 for up to 1 year. The LIC 9224 form should be kept in the child’s file for the Department’s review. An exit interview was conducted with D1 and Appeal Rights were provided by LPA. A Notice of Site Visit was posted by LPA and shall remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
