13 | On 04/07/2025, Licensing Program Analyst Katy Velazquez (LPA) conducted an unannounced complaint investigation. LPA met with Director Amy Moreno (D1). Throughout the course of the investigation, LPA conducted physical plant inspections, on-site observations, interviews, reviewed and collected documentation. It was alleged that the center is operating out of ratio. It was reported that a staff member walked out of a classroom on 04/03/2025, leaving the co-teacher over ratio with more than twelve 2 year olds. Attendance records confirm that 2 staff members were required in the Discovery Preschool classroom on this date; however, interviews did not reveal corraboration for this incident.
Based on interviews and documentation gathered, there was not a preponderance of evidence to prove or negate the allegation, therefore the allegation is UNSUBSTANTIATED. In the areas that were evaluated on 04/07/2025, no deficiencies were cited during today's inspection. An exit interview was conducted with Director Moreno and Appeal Rights were provided by LPA. A Notice of Site visit was posted by LPA and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
