13 | On 06/01/2026, Licensing Program Analyst Katy Velazquez (LPA1) and Licensing Program Anlayst Denice Pablico (LPA2) conducted an unannounced complaint investigation. LPAs arrived at the Center and observed 15 infant and toddlers being supervised by 3 staff members. LPA1 accessed Guardian to determine that all required adults were background cleared and associated to the license. LPAs met with Director Amy Moreno (D1), conducted staff interviews, and collected and reviewed documentation related to the above allegation. It was alleged that staff members did not prevent a day care child from causing harm to another daycare child.
LPAs observed incident reports that documented 7 bites in 1 classroom for the month of May. Additionally, the same child bit 3 times in 1 day. Based on interviews, file reviews, and observations conducted the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.
