Licensing Program Analyst (LPA) Gagandeep Singh met with the facility director, Paula Ritter, for a case management inspection. The facility had a complaint investigation, which was concluded on August 07, 2023 and a citation for absence of supervision was issued. Purpose of today’s inspection was to assess the civil penalties for the citation given on August 07, 2023. During today’s inspection, LPA explained the reason for the civil penalties to the director and provided the copy of the civil penalty form and appeal rights.

No deficiencies were cited today. Copy of this report was reviewed and provided to the director, Paula Ritter. Notice of site visit is posted and shall remain posted for next 30 days.
