Licensing Program Analyst (LPA) Gagandeep Singh met with the facility representative, Michelle Guarduno, regarding the complaint investigation.

During the investigation, LPA inspected the facility, interviewed staff and reviewed the records. During the investigation, LPA did not observe Notice of site visit from previous inspection (June 04, 2025). During inspection, LPA observed there broken plastic piece in the play yard, numerous amout of empty plastic cups in the play yard, debris in the classrooms and bathrooms. Based on LPA’s observations, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. California Code of Regulations, are being cited on the attached LIC 9099D. Copy of this report was reviewed and provided to the facility representative. Notice of site visit is posted and shall remain posted for next 30 days.

Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. This requirement is not met as evidenced through LPA's observations. LPA obsreved debris in the play yard and in classroom. This poses a potential Health and Safety risk to children in care.

Licensing Program Analyst (LPA) Gagandeep Singh met with the facility representative, Michelle Guarduno, regarding the complaint investigation.

During the investigation, LPA inspected the facility, interviewed staff and reviewed the records. During the investigation, it was found that there was an incident of water damage at the facility. It was found that the facility used professional service and the facility was closed for one business day. During today's inspection, LPA observed the facility bathrooms are in working condition and did not observe any leakage or hazard. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Copy of this report was reveiwed and provided to the facility representative. Notice of site visit is posted and shall remain posted for next 30 days.
