Licensing Program Analyst (LPA) Gagandeep Singh met with the facility representative, Dana Matthews, regarding the complaint investigation.

During the investigation, LPA inspected the facility, interviewed staff and reviewed the records. During the investigation, it was found that on October 28, 2025, two children were left unsupervised inside the facility. Based on LPA’s observations, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. California Code of Regulations, are being cited on the attached LIC 9099D. Copy of this report was reviewed and provided to the facility representative. Notice of site visit is posted and shall remain posted for next 30 days.

Licensing Program Analyst (LPA) Gagandeep Singh met with the facility representative, Dana Matthews, regarding the complaint investigation.

During the investigation, it was found that the facility had a child with Autism diagnosis. Per staff interviews, it was found that the child had behavior challenges. Per staff, the facility staff attempted different techniques to help the child and provide care. Per staff, the child is no longer attending this facility. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Copy of this report was reviewed and provided to the facility representative. Notice of site visit is posted and shall remain posted for next 30 days.
