Licensing Program Analysts (LPAs) Gagandeep Singh and Erwin Tjhia met with facility representative, Jessica Turner and Paula Ritter, to deliver the findings for the above allegations. Purpose of the inspection was explained.

The allegation of child's arm was puuled causing injury was investigated by the investigation bureau of the Department. During investigation, facility records were reviewed, and interviews were conducted. During investigation bureau's investigation, it was found that a child's arm was pulled and the child had to go to hospital and get medical assistance. Based interviews conducted, it was found that there has been times, when the children were left in soiled diapers. Based on observations and interviews which were conducted and record review(s), the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. See next page for deficiency cited today. Copy of this report was reviewed and provided to the director. Notice of site visit is posted and shall remain posted for next 30 days.

Director stated that the facility has conducted the training on child guidance and supervision. LPA received a copy of attendance sheet for the training. Per director, staff has been retrained on diaper needs. During today's inspection,

injury to the child. It was also found through the interviews that there has been times when children were left in soiled diapers or cloths for extended period of time. This poses an immediate health and safety risk to children in care.

LPAs observed the facility has changing log filled and shows each child being changed in timely manner.
