25 | On April 20, 2023 an unannounced plan of correction inspection was conducted by Licensing Program Analysts (LPA's) Lea Habtom and Michelle Pascual. The purpose of today’s visit is to follow up on the deficiencies assessed on April 11, 2023. LPAs met with designated staff member Lucia Vargas.
The census for today was 57 preschool children being supervised by 4 fully qualified teachers and 4 aides in the 2's, 3's and TK classrooms. Based on today's census, LPA's confirmed that the facility is in ratio with fully qualified staff to children in care. During today's inspection LPA's were able to clear the deficiency cited for the case management inspection cited on April 11, 2023.
During today's visit, it became known to LPAs that teacher Karen Baker was uncleared to work in the facility and had been working for two weeks. Due to this information a civil penalty was assessed.
In the areas that were evaluated, there was 1 Title 22 deficiencies cited during today's inspection. Exit interview conducted and report was reviewed with the Lucia Vargas. A notice of site visit was given and must remain posted for 30 days. Appeal rights were provided.

Upon receipt of this report, the Licensee shall post the Notice of Site Visit and any Licensing report documenting a type “A” deficiency. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement of Receipt (LIC 9224 form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224) Form during this visit
