It was alleged that, following the issuance of a licensing report dated 03/26/2026 documenting a Type A deficiency cited at the facility, staff did not provide each parent or authorized representative of a child in care with a copy of the licensing report.
The Director stated the licensee makes the licensing report available to parents only upon request. Statements obtained confirmed an acknowledgement document was signed without a copy of the report provided, and that in additional instances no copy of the report and no notification of the Type A deficiency was received.
The licensee produced twenty signed Acknowledgement of Receipt of Licensing Reports (LIC 9224) referencing the 03/26/2026 report, fewer than the enrollment total of thirty-five school-age children, and the Director confirmed on 05/28/2026 that no additional signed forms had been obtained.
Based on the evidence available, the preponderance of evidence standard has been met, therefore the allegation is SUBSTANTIATED. Health and Safety Code ยง1596.8595(c)(1) is being cited on the attached LIC 9099D.
One Type B Deficiency is cited on the subsequent page of this report (LIC809-D) under the California Code of Regulations, Title 22. The licensee was provided a copy of their Appeal Rights (LIC9058) and the licensee's signature on this form acknowledges receipt of these rights.
A Notice of Site Visit was given and must remain posted for 30 consecutive days.
