| 1. Staff did not ensure child's personal belongings were kept safe.
It was alleged that, on 04/02/2026, staff did not ensure that water bottles used by children in care for drinking were kept safe and that a child in care consumed contaminated liquid from a water bottle provided by the facility.
Based on observation during the facility tour, the licensee has replaced the open-access water bottle cart with a staff monitored water jug and single-use cups following the 04/02/2026 incident.
Based on statements from two staff members, the contents of the water bottle were identified by odor as possibly urine, and the substance was further identified as urine by odor and taste at the time of the incident.
Based on the evidence available, the preponderance of evidence standard has been met, therefore the allegation is SUBSTANTIATED. California Code of Regulations, Title 22, §101223(a)(2), is being cited on the attached LIC 9099D.
2. Staff are comingling kindergarten component children with school age children.
It was alleged that, during the spring break period of 03/30/2026 through 04/03/2026, staff placed children enrolled in the kindergarten program in the school-age classroom for the full operating day throughout the spring break period.
Based on interview, the Director stated the kindergarten and school-age classrooms were combined during spring break for the entire week. Staff interviews confirmed children enrolled in the kindergarten program were placed in the school-age classroom for the full operating day throughout the spring break period.
Based on record review, the school-age classroom attendance logs document the presence of children identified on the Kindergarten Roll Call Sheet as in the school-age classroom during the spring break period of 03/30/2026 through 04/03/2026.
