32 | LPA observed a current Needs and Services Plan for each infant and an Individual Infant Sleeping Plan (LIC 9227) for each child under 12 months of age. LPA observed documentation that sleep checks were conducted and documented every 15 minutes for all children under two years of age and reminded staff that napping infants must have 100% visual supervision at all times. All cribs and play yards were free of loose articles and objects, with one crib or mat for each child in care and a tightly fitted sheet; staff stated bedding is changed daily and laundered for individual use. The infant diaper changing area was clean and within arm's reach of a sink, with a one-inch-thick, vinyl-surfaced changing mat with raised sides. Toys were of age-appropriate size with no small parts. Infant bottles and food were labeled with the child's name and date and stored at safe temperatures. LPA observed staff documentation of feedings and diaper changes.
LPA reviewed the facility's records, including children's records, personnel records, administrative records, and documents required to be posted. LPA reviewed a sample of children's files and observed that each contained the required licensing documentation, including the Admission Agreement, Emergency Information/Identification, Consent for Emergency Medical Treatment (LIC 627), Physician's Report (LIC 701), immunization records, Health History (LIC 702), Needs and Services Plan, Individual Infant Sleeping Plan (LIC 9227), and acknowledgment of receipt of Personal Rights and Parents' Rights. All staff currently employed have a criminal record clearance, a health screening report (LIC 503), immunization records (Tdap, MMR, TB/PPD, and annual influenza), current Mandated Reporter Training, a signed Notice of Employee Rights (LIC 9052) and Child Abuse Reporting acknowledgment (LIC 9108), and documentation of educational background, training, and/or experience. LPA reviewed staff transcripts and observed that all staff utilized as teachers were qualified. LPA observed a current children's roster, a fire drill log reflecting drills conducted at least every six months, and all required licensing documents posted and visible to authorized representatives.
Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
