On 10/25/22, at approximately 10:00am, Application Specialist (AS) Mai Lor conducted an announced case management inspection at the above facility. AS met with Head Start Program Officer Betsy Uda and Facility Coordinator Greg Dobson for the purpose of reactivating the above license. The program will operate Monday through Friday from 8:00am-4:30pm.

AS conducted a tour of the interior and exterior of the facility. The facility appeared to be clean, safe, sanitary and in good repair. There are three preschool classrooms (classroom #3, #4, #5) and one toddler option classroom (classroom #2) that were previously licensed. A second toddler option classroom (classroom #1) was added and will not be utilized until licensed. Three new toilets and two sinks were added between classroom #3 and #4.

AS inspected the preschool and toddler option playground which appeared to be clean, safe and good repair. Areas under high climbing structure had cushion materials.

Effective 10/25/22, AS will reactivate the preschool license to serve 72 children ages 2 years to entry into Kindergarten and 15 toddlers ages 18-36 months under the Toddler Option Program. Total capacity of 87 children.

No deficiencies observed during today's inspection. Exit interview conducted and report was reviewed with Betsy Uda and Greg Dobson.
