13 | On May 14th, 2024, Licensing Program Analyst (LPA) Soleil Marx met with Director, Artastia Bolar, for the purpose of opening and closing a complaint investigation alleging that staff did not ensure infant's diapering needs were met. The purpose of today's inspection was explained.
Throughout the course of investigation, LPA made observations, reviewed records, and conducted interviews. It was determined by record review of the daily activity log, that on May 9th, 2024, there were no diaper change entries logged for an infant from 02:28pm until the child was picked up by their authorized representative at 05:17PM. Interview revealed that the infant's diaper was not changed when needed, due to staff miscommunication, which resulted in the infant sustaining a diaper rash from a soiled diaper.
Based on record review and interview, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.
