On May 14th, 2024, Licensing Program Analyst (LPA) Soleil Marx met with Director, Artastia Bolar, for the purpose of opening and closing a complaint investigation alleging that staff did not ensure infant's diapering needs were met. The purpose of today's inspection was explained.

Throughout the course of investigation, LPA made observations, reviewed records, and conducted interviews. It was determined by record review of the daily activity log, that on May 9th, 2024, there were no diaper change entries logged for an infant from 02:28pm until the child was picked up by their authorized representative at 05:17PM. Interview revealed that the infant's diaper was not changed when needed, due to staff miscommunication, which resulted in the infant sustaining a diaper rash from a soiled diaper.

Based on record review and interview, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.

The Director was informed that this report dated 05/14/2024 documents one Type A citation which shall be posted for 30 consecutive days. The director shall also provide a copy of this licensing report to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Exit interview conducted. Report was reviewed with Director, Artastia Bolar. Appeal rights were provided. Director received a Notice of Site which must remain posted for 30 days.

Director stated that the staff involved have received disciplinary action and the Director will re-train staff on diaper changing procedures. Director will provide training material to LPA Marx by POC due date.

Based on record review and interview, the facility did not ensure an infant was clean and dry at all times, by not changing an infants diaper for approximately 3 hours which resulted in a diaper rash, which poses an immediate Health, Safety, or Personal Rights risk to persons in care.
