Licensing Program Analyst (LPA) Kelly Ferrara conducted a case management inspection and met with Director Parveen Sandhu.

Upon arrival, LPA toured the facility to obtain classroom count. LPA went into a preschool classroom and observed seven children with one teacher. LPA then went outside to the preschool playground and observed 15 children with one teacher. LPA observed they were rotating children between the classroom and playground to change their diapers. LPA advised Director that the children must be in ratio both in the classroom and playground at all times.

One type A deficiency was cited. Licensee acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, Licensee shall post LIC 809 D with Type A deficiencies for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the licensee. LIC 9224 and Appeal Rights were provided.

Exit interview was conducted and Notice of Site visit was provided. LPA advised it must be posted for 30 days.

This requirement was not met as evidenced by: LPA observed 15 children in care with one staff. This is an immediate health and safety risk to children in care.

Director stated she will address the issue with the teacher today verbally. Director stated they have a staff meeting scheduled for July 16th, 2022. Director stated she will come up with an outline for the staff meeting including training on ratios and submit that to LPA by POC due date.
