13 | Licensing Program Analyst (LPA) Jennifer Velasco met with Facility Representative, Director Parveen Sandhu (Director), for the purpose of conducting an unannounced initial complaint investigation inspection pertaining to the above allegation. The purpose of today's inspection was explained to Director. During the investigation, LPA conducted interviews, observed care, and obtained relevant documentation. Based on witness statements and record review, the child sustained multiple minor injuries while in care. Based on photographic evidence and witness statements, the child (Child1) sustained multiple minor injuries, such as scratches and bruises, that were not observed or documented by staff and are therefore unexplained injuries. The preponderance of evidence standard has been met, therefore the above allegation is SUBSTANTIATED. A Title 22 deficiency being cited on LIC 9099D. Exit interview was conducted and report was reviewed with Facility Representative, Parveen Sandhu. Appeal rights were provided. Notice of site visit was given and must remain posted for 30 days.
