13 | Licensing Program Analyst (LPA) Jennifer Velasco met with Facility Representative, Parveen Sandhu (Representative), for the purpose of conducting an unannounced subsequent complaint investigation inspection pertaining to the above allegations. The purpose of today's inspection was explained to Representative. During the investigation, LPA conducted interviews, observed care, and obtained relevant documentation. Witness statements, LPA observations, and/or document reviews failed to corroborate the allegations. Though a child in care sustained minor injuries at play, witness statements and document reviews provided evidence the injuries were both explained and reported to the child's authorized representative in a timely manner. Witness statements and document reviews failed to show a child in care was left soiled for an extended time and failed to show a child was not getting their hygiene needs met. Witness statements and document reviews failed to corroborate the allegation that the facility is malodorous. Although the allegations may have happened, there is not a preponderance of evidence to prove the allegations; therefore, the allegations are unsubstantiated. Exit interview was conducted and report was reviewed with Facility Representative, Parveen Sandhu. Appeal rights were provided. Notice of site visit was given and must remain posted for 30 days.
