32 | Interviews with staff and documentation revealed that on 07/23/2026, a day-care child bit another child and broke skin. The facility had 8 children in attendance and 2 staff members present. During the time of the incident, the 8 children were being supervised by 1 staff member who observed the biting incident. The staff member reported that the incident took place so quickly that s/he was unable to prevent the bite from occurring but did provide first-aide and ice. Interviews with parents did not reveal information to corroborate the allegation.
Based on interviews, observations, documentation, and other information gathered, there was not a preponderance of evidence to prove or negate the allegation, therefore the allegation is UNSUBSTANTIATED. In the areas that were evaluated on 08/04/2026, no deficiencies were cited during today's inspection. An exit interview was conducted with Owner Anwar and Appeal Rights were provided by LPA. A Notice of Site visit was posted by LPA and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
