25 | On September 27, 2023, Licensing Program Analyst (LPA) Amanda Sutter met with Director Crystal Johnson for the purpose of an unannounced Case Management inspection. LPA observed 31 children supervised by 5 staff.
LPA arrived at the facility during nap time. Upon arrival, LPA observed two napping children in the office with the facility director. At 2:45 PM, LPA observed S1 leaving room "Shape Zone" with 4 children. S1 stated that she was taking children to the restroom. LPA then walked to room "Monkey Zone." At 2:45 PM, LPA observed 4 awake children in room "Monkey Zone." No staff member was present. S2 arrived at "Monkey Zone" at 2:47 PM. LPA asked if S2 had someone to assist with supervision, and S2 stated that they were just changing diapers. LPA then walked back to room "Shape Zone." S1 had not yet returned, and LPA observed 5 napping children in the classroom. S1 returned to the classroom at 2:55 PM. LPA then walked to class "Imaginary" and observed 5 unsupervised children napping in the classroom. Director followed LPA into the classroom and stated that she was the teacher in "Imaginary" today. Director stayed in the classroom. LPA then walked to the last preschool classroom, "Kinder College." LPA followed S3 into this classroom. LPA is unable to verify how long children in S3 were unsupervised. Director stated that S3 stepped out of the classroom to assist an adult picking up a child.
Based on the inspection, one Title 22 Deficiency has been issued on the attached LIC 809-D. The director was informed that this report dated 9/27/2023 documents one Type A citation which shall be posted for 30 consecutive days. The director shall also provide a copy of this licensing report to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. Because the citation involved an absence of supervision, an immediate civil penalty of $500 has been issued. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with Director Crystal Johnson. A notice of site visit was given and must remain posted for 30 days. Director has been provided with appeal rights. |
