On 11/14/2024, Licensing Program Analysts (LPA) Amanda Sutter met with Director Crystal Johnson for the purpose of a case management inspection.

LPA arrived at the facility at 9:50 AM. Upon arrival, LPA observed Staff 2 (S2) in the restroom supervising 12 children. LPA then went to classrooms "Shape Zone" and "Club K" to get a census for the remainder of the facility. LPA observed Child 10 (C10), Child 11 (C11), Child 12 (C12), Child 13 (C13), Child 14 (C14), and Child 15 (C15) in "Shape Zone" at this time.

At 9:55 AM, LPA observed Director supervising 5 children in the infant classroom. LPA reviewed Sign-In sheet and observed 9 children signed-in for 11/14/24. Director identified the children in her class as Child 2 (C2), Child 4 (C4), Child 6 (C6), Child 7 (C7), and Child 8 (C8). Director stated that 4 of the older children had been moved to classroom "Shape Zone". Director identified these children on the sign in log as Child 1 (C1), Child 3 (C3), Child 5 (C5), and Child 9 (C9). At 10:00 AM, LPA then went back to "Shape Zone" and observed C1, C3, C5, and C9 in this classroom. At this time, C10, C11, C12, C13, C14, and C15 had been moved to "Club K." Earlier, at 9:50 AM, LPA had observed C1, C3, C5, and C9 in the restroom with S2, who is a preschool teacher.

Staff 1 (S1) arrived at the facility at 10:05 AM and assisted Director in the infant classroom at 10:35 AM. At this time, C1, C3, C5, and C9 were moved back to the infant classroom. The total at this time was 9 infant children supervised by 2 staff. LPA observed 3 staff in the classroom at 12:05 PM and, and LPA confirmed with Staff 1 (S1) that other staff arrived at noon.

Based on the inspection, two Title 22 Deficiencies have been issued on the attached LIC 809-D. The Director was informed that this report dated 11/14/2024 documents two Type A citations which shall be posted for 30 consecutive days. The Director shall also provide a copy of this licensing report to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Licensee has been provided with appeal rights. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director Crystal Johnson.
