LPA conducted 2 out of 2 Program Officers’ interviews. LPA learned that the administration has removed S1 immediately from facility and investigated the above allegation. It was stated that there were some diapering incidents in the classroom that S1 previously worked in.
LPA conducted 2 out 2 directors’ interviews, in which LPA learned that there was no diapering log for the staff to record diapering times throughout the day. The diapering log recently started after when the facility conducted an investigation. LPA also learned that a parent complained about their child coming home with the same diaper in which it was soiled.
LPA also conducted 1 out of 2 behavioral therapists’ interviews, in which it was stated that S1 told one of them to change the child’s diaper in the restroom.
LPA conducted 3 out of 4 staff interviews regarding the above allegation. LPA learned that there were no diapering logs. LPA learned that different staff thought they were assigned to change C1’s diaper on certain days of the week and there was confusion of who needs to change C1. LPA also learned that C1 was changed once a day and right before the child transitions to another school at noon. LPA also learned that staff did not record diaper changing times.
LPA conducted 4 out of 6 parents’ interviews in which they corroborate with the above allegation. P1 stated that their child arrived at the other school in the afternoon with a pullup drenched with urine through the clothes. They have also stated that there is no diapering log to show when the children were changed daily. P3 stated that their child comes home with the same diaper and has gotten diaper rashes before. P4 stated that the staff wouldn’t change their son’s pullup up. P4 has been called twice to change the child's pullup at the facility. P8 stated that their observed an upset parent screaming at the staff about their child's diaper rash and cuts. P8 has also stated that they've observed a behavioral therapist change a child’s diaper in the restroom.
Based on the children’s interviews, there were no concerns regarding the allegation.
Based on observations and interviews, the preponderance of evidence results in substantiating the allegation the facility staff left daycare child soiled in urine and feces for a period of time. This posed as a risk to the daycare children while in care.
