25 | At approximately 10:35 AM on Tuesday November 8th, 2022, Licensing Program Analysts (LPAs) Mandie Goodwin and Josiah Gathing met with administrator, Aileen Laroco, for the purpose of an unannounced 1 year required inspection. This facility is located on the Mary Tsukamoto Elementary School site in Portable K3 for a half day program ran from 8:15am – 11:15am and 12:15pm – 3:15pm. The facility does not conduct naps. LPAs toured the facility, at which time a census of 13 preschool children supervised by two staff was observed.
A health and safety inspection was conducted in the classrooms, restrooms, food service areas, and outdoor play areas. LPAs observed the following documents are posted: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, required publications, menus, and daily schedule. Cleaning disinfectants and hazardous items are appropriately stored and inaccessible to children. The facility uses electronic sign in and sign out. Medications are stored out of reach and inaccessible to children. Furniture and equipment are in good condition, and toileting facilities are in sanitary, and operating condition. Bins for solid waste in the kitchen have tight fitting lids. The floors appeared clean throughout the facility. LPAs observed a functional smoke and carbon monoxide detector.
There is water provided inside and outside. The facility provides AM and PM snack for the children. Outdoor playground equipment and surfaces are free of loose or sharp parts. LPAs observed wood chips that are of a sufficient level to provide cushioning beneath the play structure. Outdoor shade is provided by sides of buildings. There are no firearms or bodies of water on the premises. Administrator stated there are no poisons on the premises. Fire drills are documented at least every 6 months.
A sample of children's records were reviewed. Each child's file contained an emergency card, consent for emergency medical treatment, health history, a record of immunization’s and notifications of children’s and parent’s rights. (Report continues 809-C)
