13 | Licensing Program Analyst (LPA) Gagandeep Singh met with the facility representative, Elizabeth Donkor, and Briginia Mills to investigate the above allegation. Purpose of the inspection was explained.
During the investigation, LPAs inspected the facility and interviewed staff. During the interviews, it was found that on September 01, 2023, two children were able to leave the facility unsupervised. It was found that the children were brought back to the facility by a resident in neighborhood. Based on video recording, it was found the children were left from facility unsupervised for about 12 minutes. Based on LPAs observations and interviews which were conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations are being cited on the attached LIC 9099D.
Copy of this report was reviewed and provided to the facility representative. Notice of site visit is posted and shall remain posted for next 30 days.
