Licensing Program Analyst (LPA) Gagandeep Singh met with the director, Briginia Mills, to deliver the findings for the above allegations. Purpose of the inspection was explained.

During the investigation, LPA inspected the facility multiple times, interviewed staff and the director. The investigation, it was found that there was a play structure in the infant play yard, which is labeled for 2 to 6 years old by the manufacture. Based on LPAs observations and interviews which were conducted and record review, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, are being cited on the attached LIC 9099D. See next page for citation issued today. Copy of this report was reviewed and provided to the facility representative.

The facility agreed to move the structure. During the inspection on June 20, 2023, LPA observed the structures were moved to the preschool play yard.

Licensing Program Analyst (LPA) Gagandeep Singh met with director, Briginia Mills, to deliver the findings of the above allegation. Purpose of the inspection was explained.

During the investigation, LPA inspected the facility and interviewed staff. Based on the information collected, it was found that there was a staff present in the play yard at the time of the incident. Per staff, the staff was helping another child, as one child fell and susbtain injury. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Copy of this report was reviewed and provided to the facility representative. Notice of site visit is posted and shall remain posted for next 30 days.
