25 | On 05/01/2024, Licensing Program Analyst (LPA) Jennie Tedlos conducted a case management inspection to verify corrections of 1 deficiency cited on 04/05/2024. LPA arrived at the facility and was met by Substitute Director Alejandra Paredes. LPA disclosed the purpose of the inspection and was granted entrance into the facility. LPA toured the facility and observed 57 preschool aged children being supervised 7 staff members during nap time. Criminal Record Clearances have been verified.
On 04/05/2024, the facility was cited a Type A deficiency for commingling School Aged and Preschool Aged children in the Jr. School Age Room. On 04/18/24, LPA conducted a plan of correction visit and discovered commingling was still being done in the Jr. School Age Room. Therefore, LPA issued a Failure to Correct Civil Penalty.
The facility requested and was granted a Commingling Waiver on 4/19/24 by Licensing to allow commingling of school age and preschool aged children during the first and last hour of the day.
During today's visit on 05/01/24, LPA did not observe any children in the Jr. School Age Room. The Substitute Director stated that the room is only for Preschool Aged children but is currently not being used at all. The deficiency was cleared by today's visit. A Proof of Correction (POC) letter was provided to the Substitute Director.
No deficiencies were cited today in the areas that were evaluated today. An exit interview was conducted and the report was reviewed with Substitute Director Paredes. Licensee Appeal Rights were provided by LPA. A Notice of Site visit was posted by LPA and must remain posted for 30 days. Failure to comply with posting requirements will result in an immediate civil penalty of $100. |
