13 | Licensing Program Analyst (LPA) Jennie Tedlos and Office Technician (OT) Yvonne Flores met with Acting Director, Suzie Cervantez (SC) to deliver findings of the complaint investigation regarding the above (2) allegations submitted to Licensing. There were 17 children and 2 staff during the visit.
It was alleged that staff did not provide adequate supervision to children in care. It was also alleged that the facility did not notify a child’s authorized representative of an incident. There was a concern that a child (Child 1) was injured by another child in care while under the supervision of staff and that the incident was not reported to Child 1’s authorized representative. During the course of the investigation, LPA Tedlos obtained documents, conducted observations and interviews with administration, staff, parents, and children who attend the facility. LPA observed the school-age room during the course of the investigation. LPA did not witness any concerning observations regarding supervsion. Interviews with administration, staff, and children at the center revealed conflicting statements about the incident which did not support the allegations.
