Licensing Program Analyst Katy Velazquez (LPA1) and Licensing Program Analyst Janie Davis (LPA2) conducted a field visit to the facility for the purpose of an unannounced annual inspection. LPAs disclosed the purpose of the inspection and was granted entrance into the facility. LPAs met with Director Nancy Vasquez (D1). LPA1 toured the facility inside and outside. LPAs observed 13 preschool aged children being supervised by 4 staff members. LPA1 accessed Guardian to verify that all required adults were background cleared and associated to the license. D1 confirmed that there are no Registered Sex Offenders employed at the facility. LPA1 completed the RSO profile in FAS. LPAs observed that hazardous items (disinfectants, cleaning solutions etc.) were inaccessible to children in care. Facility days and hours of operation are Monday through Friday from 6:30 AM to 5:30 PM. The facility provides breakfast, lunch and 2 snacks.

LPA1 conducted a file review prior to inspection and verified that annual fees were current. While on location, LPAs reviewed care and supervision of children, staffing ratios, medications, first aid supplies, furniture, equipment, fire drills, and drinking water. LPA1 observed all required forms to be posted. There appeared to be adequate toys and equipment available for children. LPA1 toured the outdoor play area, and the play structure appeared to be in good repair. LPA1 observed sufficient cushioning under the play structure to prevent injury.

LPA1 reviewed the sign in/out documents and observed that the children who were currently in attendance were signed-in. LPAs reviewed children’s and staff files. LPAs observed Health Screening Reports with TB test and required MMR and TDAP vaccines for present staff members. At least one staff member present today had current Pediatric CPR and First Aid (exp. 09/2024). LPAs observed current Mandated Reporter training for all staff members who were present. D1 was reminded to renew the course every 2 years. This training requirement may be met by using the Department’s Office of Child Abuse Prevention (OCAP) online training modules. The OCAP modules are free of cost and available at: http://www.mandatedreporterca.com. The training is currently provided in English and Spanish. Report continues on 809-C

Posting licensing report by child care facility or home; duration of posting; civil penalty for failure to comply; reports to be provided to parents or guardian of each child receiving services...(d)(1) A licensed child day care facility shall provide to the parents or legal guardians of each child receiving services in the facility copies of any licensing document pertaining to a conference conducted by a local licensing agency management representative with the licensee in which issues of noncompliance are discussed.
(2) Upon enrollment of a new child in a facility, the licensee shall provide to the parents or legal guardians of the newly enrolling child copies of any licensing document that the licensee has received during the prior 12-month period that pertains to a conference conducted by a local licensing agency management representative with the licensee in which issues of noncompliance are discussed.
(3) The licensee shall require each recipient of the licensing document pertaining to a conference to sign a statement indicating that he or she has received the document and the date it was received.
(4) The licensee shall keep verification of receipt in each child's file.

This facility provides Incidental Medical Services (IMS). LPA1 reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The Facility has COMPLETED TESTING WITH NO LEAD EXCEEDANCES: Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA1 verified that the lead testing was completed in accordance with the Written Directives outlined in PIN 21-21.1-CCP.

D1 was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

D1 was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

As a result of LIC 9224 not being present in all files for Department review, a deficiency was cited on a subsequent 809-D page. An exit interview was conducted, and the report was reviewed with Director Vasquez. LPA1 provided Licensee Appeal Rights to D1. A Notice of Site visit was posted by LPA1 and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
