On 5/14/2026, Licensing Program Analyst (LPA) Pa Dao Vang met with Facility Representative, Heather Johnson, for the purpose of an unannounced complaint inspection regarding the above allegations, which is that the upstairs bathroom floors appears to be in disrepair, that there is a safety hazard of glass surface in the downstairs basement where children sleep, that the classrooms are not in comfortable temperature for children, and the playground toys and equipment appears to be unsafe. The complaint report was submitted to the regional office on 3/23/2026. Upon arrival, LPAs observed 59 children supervised by 14 staff members.

On 3/25/2026 and today, LPA Vang conducted inspections, made observations, gathered documentation, and conducted interviews. LPA observed children interacting with staff indoors during nap time, guiding children to wash their hands for lunch, staff helping children to rest on mats, and staff transitioning children from outside into the classrooms.

LPA also observed the HVAC system located in the downstairs Hive classroom with air condition turned on throughout the inspections. LPA also observed the windows open in the morning. There are also an air filter and fan turned in the classroom. LPA observed the floors upstairs restroom in good condition. LPA examined the toys in the classrooms and playground which are also in working and good condition. LPA observed glass containers for the small pets at the corner of the classroom. LPA observed the children napping on the floors in the opposite side of the containers.

Based on the staff, parents, and children’s interviews, there are no concerns regarding the above allegations. In all the areas inspected during the investigation, LPA observed no violations. LPA did not learn of any violations from the interviews.

Therefore, although the alleged violation may have happened or are valid, the preponderance of evidence standard has not been met to fully prove or disprove that they did or did not occur, therefore it is unsubstantiated. There is no deficiency issued today. An exit interview was conducted, a notice of site visit, and appeal rights were provided to Facility Representative, Heather Johnson. Notice of site visit will be posted for 30 consecutive days.
