On Thursday February 15th 2024, Licensing Program Analyst (LPA) Mandie Goodwin met with Director Thuy-linh Vu for the purpose of an unannounced required inspection. Operating hours of the facility are from 7:30 am to 6:00 pm, Monday thru Friday. LPA toured the facility, at which time a census of 23 preschool children supervised by 3 staff members were observed.

A health and safety inspection were conducted in the classrooms, restrooms, food service areas, and outdoor play areas. Cleaning disinfectants and hazardous items are appropriately stored and inaccessible to children. Furniture and equipment are in good condition, and toileting facilities are in sanitary and operating condition. Bins for solid waste have tight fitting lids. The floors appeared clean throughout the facility. LPA observed smoke and carbon monoxide detectors, and fire extinguishers. Nap equipment was stored appropriately with bedding separated. LPA observed a current roster, documentation of sign in and sign outs, and documentation that facility is conducting emergency disaster drills at least every 6 months. Per record the last fire drill was conducted on 1/26/24.

Children have access to water indoors and outdoors by bringing water bottles which are refilled. Playground equipment and surfaces are free of loose or sharp parts with enough resilient material under climbing structures to absorb a fall. There are no firearms or bodies of water on the premises. Director stated there are no poisons on the premises. While LPA was touring facility, LPA observed one child leave their class and run down the hallway and around the corner into the bathroom, losing sight and supervision of the staff. The staff member stayed at the table with the group of children in the classroom. Director was in the kitchen with LPA which has an eyeline to the bathroom, however director stepped out of the kitchen as child ran through to the bathroom leaving no staff members to supervise the child. Director stated next time she will stay in the kitchen to watch children in the restroom. (Report continues 809-C)

LPA reviewed present staff files. At least one staff member present today has current Pediatric CPR and First Aid certification. LPA observed immunization records and documentation of the educational background, training, and/or experience. Upon review, all staff members had current mandated reporter trainings. LPA advised AB 1207 Mandated Reporter training must be renewed every two years and can be renewed at www.mandatedreporterca.com.

A sample of 5 children's records were reviewed. Each child's file contained an emergency card, consent for emergency medical treatment, health history, a record of immunization’s and notifications of children’s and parent’s rights.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA referred director to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.

LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA Mandie Goodwin informed Director that this report dated 2/15/24 documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.



Also, LPA Mandie Goodwin informed the Director to provide a copy of this licensing report dated 2/15/24 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. An exit interview was conducted and a notice of site visit provided to Director Thylinh Vu. Notice of site visit shall remain posted for 30 days. Appeal Rights were provided.

See LIC 9099-D for deficiencies. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
