13 | Licensing Program Analyst (LPA) Christopher Bello met with Director Phebe Thorn to close a complaint investigation, regarding the above allegation. Upon arrival, LPA observed 24 children. LPA made observations, conducted interviews and gathered documents pertaining to the investigation. It was alleged that “Staff do not adequately supervise day-care children”. A child was found by a parent without adult supervision.
Documents corroborated the allegation. Staff and parent interviews corroborated the allegation.
Director contacted Owner Julie Jenkins. Julie stated that during that incident the Teacher observed the child leaving the classroom through the facility window when the parent found the child. Julie also stated that it was only a few seconds before the Teacher went to retrieve the child in the yard.
Based on LPA's investigation, the preponderance of evidence standard has been met, therefore, the above allegations are found to be SUBSTANTIATED.
Title 22 Deficiencies have been cited on the attached LIC 809D. Upon receipt of Type A citations, facility shall post and provide copies of the LIC 809D for parents/guardians of children currently in care and for parents/guardians of newly enrolled children for the next 12 months. Facility must also keep the signed LIC 9224, Acknowledging Receipt of Licensing Reports LIC 809D in each child's files. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director Phebe Thorn.
