25 | Licensing Program Analyst (LPA) Kyrsten Williams met with facility representative, Josephine Patria Lal for an unannounced case management inspection to discuss the Unusual Incident Report (UIR) that the facility submitted on 1/17/2025. The purpose of the inspection was explained. Census included sixteen children being supervised by four staff.
The UIR stated on 1/14/2025, a preschool child was left outside on the play yard without supervision. During today’s inspection, LPA toured the facility and interviewed staff. LPA learned from interviews with staff that after transitioning children from outside back inside, a staff noticed one child in their group was not seated at the table. The staff opened the door to the playground and the child was standing right by the door. Staff stated the child was left outside for approximately one to two minutes. After discussing incident report, LPA has concluded that there was a lapse in care and supervision.
The facility representative stated the parent of the child was notified of the incident. After all children left for the day, staff received direct re-training from program specialist on effective safety and supervision. The instructor has submitted a new safety and supervision plan, a new zoning map, and implemented a roll call sheet when going outside.
A deficiency is cited on the subsequent page of this report (LIC809-D) under the California Code of Regulations, Title 22. The facility representative was provided a copy of their Appeal Rights (LIC9058) and the facility representative's signature on this form acknowledges receipt of these rights. LPA Williams informed the facility representative to provide a copy of this licensing report dated 01/23/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC9224), or other written statement, must be placed in the child's file for verification.
