25 | On Wednesday, 26 February, 2025, at approximately 11:15 am, Licensing Program Analyst (LPA) Fabian Schwartz met with Licensee, Charlotte Neal for the purpose of an unannounced annual inspection. Upon arrival, LPA observed licensee and 2 Assistants supervising 4 preschool children, 2 of which are infants. All individuals subject to criminal background review have obtained a criminal record clearance. LPA observed proper ratio and capacity was being followed. Facility hours of operation are 24/7. LPA verified that the annual fees are current. During inspection, Licensee left facility to pick up children from school, and LPA and Licensee’s 2 adult assistants were present in facility with the 4 children present at beginning of inspection while Licensee was away from facility. Licensee returned to facility with 2 school age children bringing total census for inspection to 6.
A health and safety evaluation was conducted in all areas accessible to children. Facility is a 4 bedroom, 3 bathroom multi-story home with two sets of staircases. Off-limit areas include: Entire Upstairs and Garage. Licensee acknowledged that children may never enter these off-limit areas. Upon arrival, LPA observed set of stairs which go to off-limits, upstairs portion of home were appropriately barricaded, but second set of stairs which goes to lowest level of facility were not appropriately barricaded. Due to having children in care under 5 years of age all stairs shall be fenced or barricaded. LPA observed that the facility is clean, sanitary, and in good repair. LPA observed a functioning smoke detector, carbon monoxide detector, and a full 2A10BC fire extinguisher. The facility has adequate toys that appear to be safe for children to use. The licensee stated there are no weapons or poisons in the home. LPA observed that there is a fenced backyard with no pool.
LPA reviewed 3 children’s files which were observed to not be complete. Licensee did not have files for some children in care that were present in facility. Required postings were seen on east wall in front entryway and the children’s roster was observed. LPA did not observe infant sleep plan (LIC 9227) and 15-minute observation checks for napping infants. The facility has record of conducting fire drills at least every 6 months with the last fire drill being conducted on 13 July 2024. Licensee’s CPR/First Aid card expires 1 June 2026. Licensee’s Mandated Reporter certificate expires 3 October 2026. Licensee understands that trainings must be completed every two years. PAGE 1. REPORT CONTINUES ON LIC809-C |
