On 09/02/2025, Licensing Program Analyst Katy Velazquez (LPA1) and Licensing Program Analyst Deborah Khashe (LPA2) conducted an unannounced complaint investigation to deliver the findings for the above allegations. LPAs arrived at the Family Childcare Home (FCCH) and were granted entrance. LPAs observed 1 infant and 2 preschool aged children. Also present in the FCCH was L1’s spouse. LPA1 accessed Guardian to determine that all required adults were background cleared and associated to the license.
Throughout the course of the investigation, LPA1 and LPA2 conducted physical plant inspections and observations. LPA1 conducted interviews and reviewed and collected documentation. It was alleged that Licensee inappropriately disciplined a daycare child while in care, yells at day care children, and intimidates children while in care.
Interviews revealed that L1 spanked a child with her hand during day care hours and spanked another day care child with an object. Observations and interviews revealed that L1 yelled at children in care and yelled at others in front of children in care.

Interviews and observations revealed that L1 has used words to intimidate children in care and used authority to deny children’s personal rights such as withholding water, requiring children to stay on nap mats, requiring children to sit down longer than developmentally appropriate.
Based on interviews, file reviews, and observations conducted, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. Type-A deficiencies was cited on a subsequent 9099-D page(s). L1 acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, licensee shall post LIC 9099-D with Type A deficiencies for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the licensee. An exit interview was conducted, and the report was reviewed with L1. LPA1 provided Licensee Appeal Rights to L1. A Notice of Site visit was posted by LPA1 and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Licensee Johnson will watch the training video titled, "Children's Personal Rights in Child Care" on the CDSS website. Licensee will email LPA1 a signed statement that she watched the video in entirety by 5:00 PM on 09/03/2025.
