Licensing Program Analyst Katy Velazquez (LPA) and Licensing Program Analyst Elizabeth Santiago (LPA2) conducted a field visit to the facility for the purpose of an unannounced annual inspection. LPAs disclosed the purpose of the inspection and were granted entrance into the facility. LPAs met with the Director Rhonda Hensley (D1) and Site Supervisor Katelyn Endicott (D2). LPA toured the facility inside and outside. LPA observed 4 infants being supervised by 1 staff member. LPA accessed Guardian to verify that all required adults were background cleared. LPA observed that hazardous items (disinfectants, cleaning solutions etc.) were inaccessible to children in care. Facility days and hours of operation are Monday-Friday from 7:00 AM to 6:00 PM.

LPA conducted a file review prior to inspection and verified that annual fees were current. While on location, LPA reviewed care and supervision of children, staffing ratios, medications, first aid supplies, furniture, equipment, fire drills, and drinking water. LPA observed all required forms to be posted. Nap Mats are available for infants over 12 months old. A Needs and Services Plan was on file for each infant/toddler enrolled. LPA observed a refrigerator on location to store bottles should it be required; LPA did not observe any bottles currently stored.

LPA discussed the safe sleep regulations with D1 and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA discussed the Infant Sleep Regulations with D1. LPA discussed the requirement to check and log infant napping every 15 minutes. LPA did not observe any Individual Sleeping Plan/LIC 9227 as the facility only has children over 12 months currently enrolled. LPA also informed D1 of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended that s/he register all infant devices with the CPSC to be notified of any recalls on purchased equipment.

LPA observed a changing table within arm's reach of the sink. There appeared to be adequate toys and equipment available for infants. LPA toured the outdoor play area, and the play area is not co-mingled with other age groups. LPA observed sufficient cushioning under the play equipment to prevent injury.

LPA reviewed the sign in/out documents and observed that the children who were currently in-care were signed-in. LPA reviewed children’s and staff files. LPA observed Health Screening Reports with TB test and required MMR and TDAP vaccines for staff members. A staff member present today had current Pediatric CPR and First Aid training which will expire on 08/01/2024. LPA did not observe current Mandated Reporter training for staff members. LPA discussed Mandated Reporter Training with D1. Health and Safety Code 1596.8662 requires all licensed providers, applicants, directors and employees complete training as specified on Mandated Reporter duties and to renew training every 2 years. Volunteers are encouraged but not required to take the training. This training requirement may be met by using the Department’s Office of Child Abuse Prevention (OCAP) online training modules. The OCAP modules are free of cost and available at http://www.mandatedreporterca.com. The training is currently provided in English and Spanish. D1 was reminded to renew the course every 2 years.

Incidental Medical Services (IMS) policy was discussed with D1. LPA requested that D1 submit an IMS plan to provide care for children with epipens and or inhalers. For IMS information see PIN 22-02- CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The Facility has COMPLETED TESTING AND IS IN THE PROCESS OF SUBMITTING THE REQUIRED DOCUMENTATION: Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

D1 was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. LPA informed Director Hensley that this report dated 09/14/2023 contains 1 Type A citation which shall be posted for 30 consecutive days as there is an immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA informed L1 that she must provide a copy of this licensing report dated 09/14/2023 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day, or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

LPA conducted an Exit Interview and the report was reviewed with D1. LPA provided Licensee Appeal Rights to D1. A Notice of Site visit was posted by LPA and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
