An unannounced collateral inspection of the facility was conducted by Licensing Program Analyst Jennifer Velasco (LPA), who met with Facility Representative Michelle Lucas (M1). LPA conducted an interview with a witness regarding a current complaint investigation at a separate licensed facility.

During today's visit, LPA toured the facility and observed three staff members providing care to 17 children on the preschool license (343621079) and two staff members providing care to four children on the infant license (313623348).

This report was reviewed with M1 and an exit interview was conducted. LPA provided M1 with a Notice of Site Visit, which must remain posted for 30 days.
