13 | Licensing Program Analyst Jennifer Velasco (LPA) met with Licensee Marlene Piedmont (L1) to deliver findings. LPA toured the facility, including all activity and classroom spaces, restrooms, food service and outdoor play areas. Census included 10 children being supervised by L1 and an assistant. L1 was reminded never to exceed the conditions, limitations, and capacity specified on the license. Facility hours of operation are Monday through Friday from 6:00 AM to 5:00 PM.
It was alleged: Provider leaves day care children unattended due to inadequate staffing, Provider is under the influence while caring and supervising day care children, Provider leaves day care child soiled for an extended period of time causing day care child sustaining rashes, Day care child sustained multiple bug bites while in care, Provider does not clean or sanitize day care toys, Provider does not meet day care child's needs. During the investigation, LPA observed staff provide care to children, conducted interviews, and reviewed documents. Witness statements, LPA observation, and facility documentation reviews failed to corroborate the allegation(s). The preponderance of evidence standard has not been met; therefore, these allegations are unsubstantiated. An exit interview was conducted, and a notice of site visit provided. Notice of site visit shall remain posted for 30 days. |
