32 | LPA’s interviews also revealed that there had been some concern with how prior staff had spoke to the children in care, but they had been let go for their conduct and were no longer employed at the center.
It was also alleged that the “facility staff are not taking appropriate measures to prevent children from harming each other.” Again, staff interviews and LPA’s observations of staff interactions with children in care did not reveal a concern relating to how the staff are monitoring the children and preventing them from harming each other.
Finally, it was alleged that the “facility staff are not following reporting requirements when children are injured.” LPA’s review of pertinent documentation did not reveal a concern regarding the center not following reporting requirements.
Based on LPA’s observations, conflicting statements and a lack of clear corroborating evidence, the above allegations could not be substantiated or dismissed. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the finding is UNSUBSTANTIATED.
An Exit interview was conducted in which the report was reviewed and discussed with the Director. LPA provided a copy of the report and Appeal Rights to Director. A Notice of Site visit was posted by LPA and Director understands it must remain posted for 30 days. |
