On Tuesday, 13 August, 2024, at approximately 10:30am, Licensing Program Analyst (LPA) Fabian Schwartz met with Licensee, Denise Stanko, for the purpose of an unannounced annual inspection and capacity increase. Upon arrival, LPA observed licensee and assistant supervising 4 children. All individuals subject to criminal background review have obtained a criminal record clearance. LPA observed proper ratio and capacity was being followed. Facility hours of operation are Everyday from 5:00am-6:30pm. LPA verified that the annual fees are current.

A health and safety evaluation was conducted in all areas accessible to children. Facility is a 4 bedroom, 2 bathroom two-story home. Off-limit areas include: Downstairs Bedrooms, Entire Upstairs, and Garage. Licensee acknowledged that children may never enter these off-limit areas. LPA observed that the facility is clean, sanitary, and in good repair. LPA observed a functioning smoke detector, carbon monoxide detector, and a full 2A10BC fire extinguisher. During annual inspection, LPA observed bottle of diluted bleach on kitchen counter which would be accessible to larger children in care. During inspection, Licensee immediately removed bleach solution and stored it in locked, off-limits garage. The facility has adequate toys that appear to be safe for children to use. Cleaners and poisons are stored in locked, off-limits garage which is inaccessible to children in care. The licensee stated there are no weapons in the home. LPA observed that there is a fenced backyard with no pool.

Because the licensee rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC9149).
