13 | Licensing Program Analyst Erwina Pascual-Golamco (LPA) met with Licensee, Ladan Kavan, to deliver findings. LPA toured the facility, including all activity and classroom spaces, and restrooms. LPA observed 11 preschool children supervised by two staff.
Throughout the course of the investigation, LPA toured the facility, observed staff provide care to children, requested documents and conducted interviews. LPA interviews and statements were inconsistent to corroborate the allegations Child was confined in a room, Facility carpet/flooring is unsanitary, and Facility over ratio. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.
Exit interview was conducted and report was reviewed with Licensee, Ladan Kavan. Appeal rights were provided, and a Notice of Site visit was given to Licensee, who will post it where visible to parents/guardians for 30 days. |
