Licensing Program Analyst (LPA) Kelly Ferrara met with Owner Kelli Vaccaro for an unannounced annual inspection on July 22, 2021. At 9:35 AM, LPA toured the facility including all activity and classroom spaces, restrooms, food service and outdoor play areas. Appropriate ratio and care and supervision were observed during today's inspection. Census included 63 children being supervised by ten staff. Owner was reminded never to exceed the conditions, limitations and capacity specified on the license. Facility hours of operation are Monday through Friday from 6:30 AM to 6:30 PM.

Classrooms all appeared clean including the carpets and floor. Chemicals were kept inaccessible to children on a high shelf. Playground equipment and surfaces were inspected and are in good condition with enough resilient material under climbing structures to absorb a fall. Uncontaminated drinking water was readily available to children both indoors and outdoors. Bathrooms were clean and all sinks and toilets were in operating condition. There are no firearms or bodies of water on the premises and the facility has at least one functioning smoke and carbon monoxide detector. Menus were posted for AM and PM snack and the children are provided lunches. LPAs observed all required licensing documentation was posted for parental review. LPA observed that parents are signing their children in and out of the center electronically. LPA discussed current Covid guidelines with the Director.

A sample of children and staff files were reviewed. Each child's file had the required licensing documentation. At 11:45 AM, during the staff file review, LPA observed that five out of 15 total staff files were missing a current Mandated Reporter certificate. LPA reviewed transcripts and observed that one staff being utilized as a qualified teacher was not qualified based on course work. At least one staff had a current Pediatric CPR and First Aid certification that expires in September 2022. Each classroom is equipped with First Aid equipment.

Incidental Medical Services (IMS) policy was discussed with the facility representative. For additional IMS information, director was advised to see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226.The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm.

The facility evaluation report was reviewed and discussed with the Owner. Owner was encouraged to the visit the department's website at WWW.CCLD.CA.GOV for information regarding child care updates, PINs, forms, regulations and legislation pertaining to child care centers.

A notice of site visit was provided and should remain posted for a period of 30 days for parental review.

One Type A Title 22 deficiency was cited on the subsequent page of this report. Owner acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, Director shall post LIC 809 D with Type A deficiencies for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the licensee. LIC 9224 and Appeal Rights were provided.

Owner rearranged teachers so that Staff #1 was not being utilized as a qualified teacher. Owner stated she will come up with a checklist to ensure each staff have all required units listed on their transcript. Owner shall submit a staff schedule to LPA verifying there is one qualified staff per classroom. Checklist and schedule are due by POC due date.

This requirement was not met as evidenced by: LPA observed Staff #1 being utilized as a qualified teacher with an aide, however staff file review revealed Staff #1 was not qualified. This poses an immediate risk to health and safety of the children.
