On 2/12/2026 at approximately 10:20 AM, License Program Analyst (LPA) Pa Dao Vang conducted a complaint investigation and met with Director, Huimin Huang (Mindy). It was alleged that the daycare child sustained unexplained injuries. LPA observed 18 children supervised by 5 staff.

On 12/8/2025 and 2/12/2026, LPA made observations, conducted interviews, gathered documentation, and reviewed a file. Throughout these days, LPA observed the staff conducting indoor activities with the children. LPA also observed meal service, nap times, and the classrooms transitioning into different environments. Based on interviews, LPA learned that on 11/21/2025, Director followed up with a parent about an incident regarding C1’s bloody nose. Director stated there were substitutes working in the classroom when C1 had a bloody nose. Staff interviews revealed that the substitutes didn’t know about the bloody nose. Based on parent interviews, one parent revealed that her child sustained an injury while in care and facility staff were not able to explain how the injury occurred. Continue on LIC9099-C...

This is an amended report.

Based on observations and interviews, the preponderance of evidence results in substantiating the allegation of the daycare child sustained unexplained injuries.

The Title 22 deficiencies are being cited on LIC 9099-D pages. LIC9224 and Appeal Rights were provided. An exit interview, appeal rights and a notice of site visit were provided to Director Huimin Huang. A Notice of Site Visit posted. Failure to comply with posting requirements can result in a $100 penalty.

On 2/12/2026 at approximately 10:20 AM, License Program Analyst (LPA) Pa Dao Vang conducted a complaint investigation and met with Director Huimin Huang (Mindy). It was alleged that the daycare child sustained unexplained injuries and the Staff are not following reporting requirements.

On 12/8/2025 and 2/12/2026, LPA made observations, conducted interviews, gathered documentation, and reviewed a file. Throughout these days, LPA observed the staff conducting indoor activities with the children. LPA also observed meal service, nap times, and the classrooms transitioning into different environments. LPA observed multiple staff guiding and supervising children in the restroom. LPA also observed documentation of diaper changing logs throughout the day. LPA also observed daily exchange forms that the staff provides to the parents regarding the child’s diapering, meal service, and activities throughout the day.

Through the staff’s interviews, LPA learned that the children’s diapers are changed before and after transitions into a new environment. Based on Director and staff’s interviews, LPA also learned that there was no incident with C1 on 11/20/2025. Through the file review, LPA did not observe a written report from the staff about the incident that took place on 11/20/2025. The parent’s interview revealed no corroboration with the above allegations.

Therefore, although the alleged violations of the staff are not following reporting requirements, and staff left child in a soiled diaper for an extended period of time may have happened or are valid, the preponderance of evidence standard has not been met to fully prove or disprove that they did or did not occur, therefore it is unsubstantiated. There is no deficiency issued today. An exit interview was conducted, a notice of site visit, and appeal rights were provided to Director, Huimin Huang. Notice of site visit will be posted for 30 consecutive days.
