On 4/7/2026, License Program Analyst (LPA) Pa Dao Vang conducted a complaint investigation and met with Director, Huimin Huang. It was alleged that the facility staff handled children in a rough manner. There were 22 children supervised by 6 staff members.

On 2/14/2026, the facility submitted an LIC624 Unusual Incident Report regarding two occasions where S1 was observed redirecting children's behavior using a strong tone with two different children. The Director stated that S1 was removed from the facility, while administration was conducting an investigation. LPA learned that S1 has been transferred to several different SETA facilities.

On 2/18/2026, 3/17/2026, and today’s inspection, LPA made observations, conducted interviews, and gathered documentation. Throughout these days, LPA observed the staff conducting indoor and outdoor activities with the children. LPA also observed meal service, nap times, and the classrooms transitioning into different environments. LPA also observed different staff guiding and supervising children in the restroom. LPA Vang conducted 2 out 2 director interviews, in which stated that they’ve observed S1 being “sharp” with her tone of voice with children. In the staff interviews, there are 3 out of 5 staff members confirming that they’ve witnessed S1 "yelling, using a sharp tone of voice, scaring the C2 to nap, and picking up C1 in a rough manner to sit for circle time." These staff also stated that these incidents made them feel “upset, uncomfortable, and sad.” In the children’s interviews, 1 out of 2 children specified that S1 grabbed the back of his neck at nap time because he was trying to drink water. C2 stated that S1’s voice is “scary.” C2 also stated that it makes him “sad” when S1 talks to him. In the parents’ interviews, 2 out of 4 stated corroborate the allegation of a facility staff handling children in a rough manner. According to P1, there was a staff member that yells at the children. P1 stated that C2 specified that S1 “grabbed and pulled the back of his neck.”

Based on observations and interviews, the preponderance of evidence results in substantiating the allegation a facility staff handled children in a rough manner. This posed as a risk to the daycare children while in care.

The Title 22 deficiency is being cited on LIC 9099-D pages. Facility representative acknowledges, that for TYPE A DEFICIENCIES ONLY upon receipt, facility shall post LIC 9099-D with Type A deficiency for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the Director, Huimin Huang. LIC9224 and Appeal Rights were provided. An exit interview, appeal rights and a notice of site visit were provided to Director, Huimin Huang. A Notice of Site Visit posted. Failure to comply with posting requirements can result in a $100 penalty.

Based on the Directors, staff, parents, and a child's interviews, it was found that S1 was handling children in a rough manner and yelling at them while in care. This posed an immediate health, safety or personal rights risk to persons in care.
