25 | At 10:30 a.m. on Thursday, January 26th, 2023, Licensing Program Analyst (LPA) Karyn Guerra met with Assistant, Mylinh Garner, for the purpose of an unannounced, required - 1 year inspection. Upon arrival, LPA observed a census of 7 children including 1 infant, 5 preschoolers, and 1 child aged 6. Licensee was not present during inspection. It was stated that they would return to the facility either later today or tomorrow. Hours of operation are 6:30 a.m. - 6:00 p.m., Monday thru Friday. Off limit rooms consist of Bedroom #4 and Bedroom #5. Assistant understands that child care children must not enter the off limits areas.
All individuals subject to criminal background review have obtained a criminal record clearance. Assistant was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
A health and safety inspection was conducted in all areas accessible to children. LPA observed the required postings, a working phone, 2A10BC fire extinguisher, and functioning smoke and carbon monoxide detectors. Assistant stated that there are no weapons nor poisons in the home. Fireplace is appropriately barricaded to prevent access by children. Fire Drills have been conducted and documented. LPA observed a shaving razor in an unlocked drawer in the bathroom. This poses a potential risk to the health and safety of children in care.
LPA reviewed staff and children's files LPA observed current in person EMSA CPR and First Aid certification for Licensee, expiring 6/27/2023. Staff file for Assistant was missing. It was stated that Assistant also took CPR coursework in June. LPA observed immunization records, affidavits, additional children in care form, emergency identification, parents rights, and consent for emergency medical treatment in child files.
