This is an amended report.
Licensing Program Analysts (LPAs) Andrea Cortez and Sala Vang met with Facility Representative, Kellie Rodriguez to conduct an unannounced subsequent complaint investigation inspection pertaining to the above allegation. The purpose of today's inspection was explained. During the investigation, LPAs conducted interviews, observed care, and requested relevant documentation.

Interviews reflected ongoing concerns related to facility cleanliness and the availability of routine cleaning supplies. Interviewees describe unpleasant indoor odor, and a tattered activity rug with brown spots. Others indicated that cleaning supplies are not always be readily available. The preponderance of evidence standard has been met; therefore, the allegation is substantiated. Exit interview was conducted and report was reviewed with Facility Representative, who was provided with Appeal Rights. Notice of site visit was provided and must remain posted for parental review for 30 days.

Licensing Program Analysts (LPAs) Andrea Cortez and Sala Vang met with Facility Representative, Kellie Rodriguez to conduct an unannounced subsequent complaint investigation inspection pertaining to the above allegations. The purpose of today's inspection was explained.

During the investigation, information gathered from interviews indicated that some individuals observed conditions that contribute to the food service, sanitation, and pest control of the facility. LPA’s observations, document reviews, and witness statements failed to corroborate the allegations mentioned above.

The preponderance of evidence standard has not been met; therefore, the allegations are unsubstantiated.
Exit interview was conducted and report was reviewed with Facility Representative, who was provided with Appeal Rights. Notice of site visit was provided and must remain posted for parental review for 30 days.

The Director will conduct monthly health and safety inspections of the facility. A cleaning supply inventory will be maintained and reviewed weekly. Staff will be reminded of cleaning and sanitation procedures and the responsibilities to promptly report facility maintenance concerns.

Based on observations and interviews, the licensee did not comply with the section cited above. There is an unpleasant odor, insufficient cleaning supplies, and a tattered carpet with brown spots, which poses a potential health, safety or personal rights risk to persons in care.
