13 | An unannounced subsequent complaint investigation inspection was conducted by Licensing Program Analysts Mandie Goodwin and Jennifer Velasco (LPAs), who met with Director Maya Patel (D1). It has been alleged a child (C1) was locked in a closet. D1 denied the allegation and stated no child has ever been locked in anything at the facility.
During today’s inspection, LPAs toured the facility and observed 6 classroom staff providing care to 39 children. During the investigation, LPAs toured the facility inside and outside, observed facility staff provide care to children, conducted witness interviews, and requested and reviewed pertinent documents and materials. Witness statements, document reviews, and LPA observations failed to corroborate the allegation that facility staff locked a child in a closet. Based on interviews and facility documents, the preponderance of evidence standard has not been met; therefore, the above allegation is found to be UNSUBSTANTIATED. This report was reviewed and discussed with D1. Appeal Rights were provided and exit interview was conducted. All licensing reports are public information and must be made available upon request for at least three years. D1 was provided with a Notice of Site Visit to be posted in the facility for 30 days. |
