On May 19th 2025 Licensing Program Analyst (LPA) Mandie Goodwin and Licensing Program Manager Seychelle De Luca met with director Maya Patel to close a complaint investigation. Upon arrival 16 preschool children were present supervised by 5 staff members across 3 classrooms.
It was alleged that staff left daycare child in soiled clothing, that staff left the daycare child unattended during a school drop off, and that staff yelled at a daycare child. Through the course of the investigation LPA made observations and conducted interviews. Through interviews it was learned that a staff member (S1) drove a child (C1) to school and C1 had an accident and was in soiled clothing. One interview stated that the staff member smelled that C1 had an accident, but S1 thought the child had passed gas. The child was dropped off at elementary school, where interviews stated that the elementary teachers found the child outside, unattended, and C1 was in soiled clothing. Interviews revealed that the child was dropped off at the curb of the school without ensuring the child was in the care of their teacher. Interviews also stated that S1 used profanity in the presence of a daycare child and was "mean." This is considered a violation of the child's personal rights. This is a repeat violation- a civil penalty of $250 was assessed. Continued on 9099-C

Based on interview conducted the preponderance of evidence standard has been met; therefore, the above allegations are substantiated. LPA Mandie Goodwin informed director that this report dated 5/19/25 documents two Type A citations which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPA Mandie Goodwin informed the Director to provide a copy of this licensing report that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview with director Maya Patel was conducted and appeal rights were provided.
