25 | On Tuesday, 26 April, 2024, at approximately 2:00pm, Licensing Program Analyst (LPA) Fabian Schwartz and Licensing Program Manager Amanda Blesi met with Licensee, Lakia Parker, for the purpose of an unannounced annual inspection. Upon arrival, LPA observed licensee supervising 5 children. All individuals subject to criminal background review have obtained a criminal record clearance. LPA observed proper ratio and capacity was being followed. Facility hours of operation are Monday through Friday, 7:00 AM to 6:00 PM. LPAs verified that the annual fees are current.
A health and safety evaluation was conducted in all areas accessible to children. Off-limit areas include: Entire Second Floor. Licensee acknowledged that children may never enter these off-limit areas. LPA observed that the facility is clean, safe, sanitary, and in good repair. LPA observed shampoos in an area accessible to children, which licensee moved to off limits area. LPA observed a functioning smoke detector, carbon monoxide detector, and a full 2A10BC fire extinguisher. The facility has adequate toys that appear to be safe for children to use. The licensee stated there are no weapons or poisons in the home. LPA observed that there is a fenced pool at the housing complex which Licensee states daycare does not use. There is no backyard.
LPA reviewed 3 children’s files which were not observed to be complete. Children’s files were missing Immunization Records. Required postings were seen at immediate right-hand area from door entrance and the children’s roster was observed. LPA observed no infant sleep plan (LIC 9227) or 15-minute observation checks for napping infants. Licensee stated they have not had infants before and current infant on file had yet to start care. The facility has no record of conducting fire drills at least every 6 months. Licensee’s CPR/First Aid card expired on November 2021. Licensee’s Mandated Reporter certificate expired on 03/28/22. Licensee understands that trainings must be completed every two years.
