On Friday, May 15, 2026, Licensing Program Analysts (LPAs) Payenda Seddiqi and Kyrsten William met with Director Carrie Nguyen regarding the above allegation. Upon arrival, LPAs observed 28 preschool children being supervised by 6 staff.

It was alleged staff do not ensure playground equipment is in good repair. Through the course of the investigations, LPAs toured the facility, including all areas accessible to children, observed staff providing care to children, and conducted interviews. Observations revealed that the equipment is in good condition. Based on interviews, the teachers are trying to teach children to use the equipment appropriately. They do a morning walk through every day to make sure they are in good use, and they order supplies once a month.

It was alleged staff did not ensure children had water available during outside play. Through the course of the investigations, LPAs also toured the facility, including all areas accessible to children, observed staff providing care to children, and conducted interviews.

Observations revealed that facility provides water to children while playing outside. Also, there are water fountains throughout the facility. Interviews revealed that children are allowed to bring water bottles with them, and they have just sent out a newsletter encouraging parents to bring a labeled bottle for their child.

It was alleged staff left children unsupervised. Throughout the course of the investigation, LPAs conducted interviews. Through interviews, LPAs learned that there was one incident where a school-age teacher left the classroom and center after receiving an emergency phone call. It was reported that during this time there were two CCC subs were present in the room to assist with supervision of children. LPAs learned children were not left unsupervised during this incident. Additional interviews conducted with staff and parents did not reveal concerns regarding supervision.

LPAs interviews and statements were inconsistent to corroborate the above allegations. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.

Exit interview conducted and report was reviewed with Director Carrie Nguyen. A notice of site visit was given and must remain posted for 30 days.
