On Wednesday, May 28, 2025, Licensing Program Analysts (LPAs) Amanda Sutter and Payenda Seddiqi met with Director Carrie Nguyen to deliver findings regarding the above allegation. Upon arrival, LPA observed 35 preschool children supervised by 5 staff and 19 toddler children supervised by 5 staff. It was alleged that the facility operates out of ratio.

LPAs reviewed staff files and confirmed that the following teachers are fully qualified staff: Staff 1 (S1), Staff 3 (S3), Staff 13 (S13), Staff 15 (S15), and Staff 19 (S19). Staff 11 (S11) has 6 units and is currently enrolled. The following staff are all aides with 0-3 units: Staff 7 (S7), Staff 8 (S8), Staff 10 (S10), and Staff 18 (S18).

On 5/28/2025 at 11:38 AM, LPAs observed S7, S8, S10, S18, and S19 supervising 22 childrenin the toddler classroom. Director stepped in to break S19 at this time. S19 was the only qualified teacher in the classroom at the time. CONTINUED ON LIC9099-C

At 12:25, LPAs observed Teachers S1 and S3 on the playground with 26 children. LPA confirmed with S1 that the class had been over ratio by 2 children for about 10 minutes.

Based on the observations and interviews, the preponderance of evidence standard has been met; therefore, the above allegations are SUBSTANTIATED.

One Title 22 Deficiency has been issued on the attached LIC 9099-D. The Director was informed that this report dated 5/28/2025 documents two Type A citation regarding the facility being out of ratio and unqualified staff supervising children. The Type A citations shall be posted for 30 consecutive days. The Director shall also provide a copy of this licensing report to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Director has been provided with appeal rights. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Carrie Nguyen.
